In this section there is the deanery guidance on appraisal for trainees on the VTS. Appraisals should be conducted once a year for all doctors in training. It is the responsibility of the doctor themselves to ensure that they are appraised.
Form 4 should be sent to the PCT appraisal lead, and summary of in house assessment to the PDs.

Your appraisal will be included in your end-of-year review for this reason you must ensure that you have an up-to-date PDP.
Your appraisal will be conducted after you have received the results of your panel review towards the end of the year. This will be conducted by your Trainer.
The bulk of the documentation will be your e-portfolio and it is likely a new section for appraisal will be added during the course of the year. You will have to provide a completed form 4. It would be advisable to look at the NHS Appraisal Toolkit at this time for future use.
